A procedure that could not have been performed on this patient is the clearest error a bill can contain. Charges land on the wrong account when two patients share a room, a surname or a similar record number. The examples are sometimes this obvious and often not. Read each line and ask one question: did this happen to this patient? New parents, tired and relieved, are among the least likely people to check.
